Insurance Forms

The office staff at Academy Massage Therapy are happy to assist you with your private insurance policy. All policies differ in coverage amounts and certain policies require a physician prescription prior to your treatment. We are experienced at dealing with most types of policies, and it is important that you are aware of the details of your plan. Please call us for more information.

Your insurance provider may requested a deductible to be paid to them prior to treatment, they may also ask for a prescription from your MD. All plans vary in coverage for example, if 80% of your Massage Therapy treatment is covered by your Insurance Provider, then you will be required to pay 20%. Please be aware of YOUR personal insurance plan and it?s requirement as they may change.

Currently we offer direct billing for following three providers. We require 2 business days prior to your massage appointment to create your profile for direct billing.

For your convenience, we have also provided some other common insurance company claim forms. You can print the appropriate form, fill it out and submit it to your provider for the appropriate refund.

 

Patient History Form

When you first arrive at Academy Massage, one of the staff will require that you fill out a Patient History Form. For your convenience, click here to fill out the form electronically that you can print and bring with you for your first visit
